I have arthritis in my fingers, and the vertical mouse leaves my hand feeling very comfortable and relaxed. An unexpected side effect is my whole arm feels more comfortable, since palm down is an unnatural position for the hand. Palm facing center is more natural. It's important to get the right size. This mouse lets me work with out aggravating my carpel tunnel problems. But I dropped it only once on carpet, and it broke! The plastic arm is a lever with very fragile plastic tabs that connect the arm to the base. One of these plastic tabs broke and is rattling around inside the base. The arm is now loose. I bought the 3M ergonomic mouse because of excessive pain and swelling in my right index (mouse) finger for several weeks. Since using the mouse for about 3 weeks the pain is gone. The thumb mouse button is easy to use, but aiming and moving the mouse around on the page is harder to master. It can be done though with patience - but the first week is rough. 9 years ago I was having to leave work early because using a "normal" mouse was causing me so much inflammation and pain. One day in a store I saw a "3M ergonomic mouse" on a shelf and I bought it instantly. 9 years later, that same mouse (not a laser mouse like this one, rather, a traditional mouse with a roller ball in it) continues to provide me with a pain free way to do my job, and is easily one of the best purchases I have ever made.
The only thing I would caution you (and if you look at the 1 start reviews you'll see this) is that it does make heavy use of your thumb, so it's possible that clicking with your thumb might eventually start to cause pain. I've never had any problems with it myself however.

First, this mouse has two sizes. I am 5'8, male, and have average sized hands. The small/medium sized mouse was too small for my comfort. Second, I do not like having the main buttons on the top. Those who suffer from some type of RSI are generally susceptible to other types. Using my thumb so much gave me trigger finger. I stopped using the mouse and went back to my old mouse.
