I purchased this item and it would not work when the headphone was plugged in all the way...the sound was muffled. But it worked fine when the headphone was plugged in 3/4 of the way. Eforcity sent me a replacement at no charge but unfortunately the replacement doesn't work at all.Digital camera Review: Great if you like music only in your right ear!

This device is junk - poor quality and does not work.
This adapter worked with my iphone and my pricey Ultimate Ears headphones for about a week before the left earphone cut out entirely. I was on a 10-hour flight and was able to get it to work on occasion only by firmly applying sideways pressure to the jack. Anytime I let go the left ear cut out again.
To confirm it was the adapter, I stopped at radioshack back in the U.S. and got a Belkin for $10 which works great. I do not like the rigid Belkin model as much in design as this flexible cord, but this thing is useless. It was cheap enough that I will not bother to send it back, but don't waste your time or money.
In addition, I waited over a week for the item to arrive. When I inquired to the company about delivery date, they sent me a standard reply that shipping could take 7-14 days, even though it was stated to be "In-Stock" and I selected standard 5 day shipping.

If you're like me and have a 1st Generation iPhone then this adaptor is nice to have if you want to use normal headphones, earbuds, or if you want to run your music through a sound system. I don't know why Apple didn't do flush headphone jacks, but this item does a great job of solving that problem! It's a little long for an adapter, but the earbuds are short anyway. This only helps with that! Thanks!
